The adapter will be needed if you have an adapterplate between the engine and gearbox so that the release bearing will reach the clutch.
Note. The adapter is meant to fit on a flexplate, not directly to the crankshaft.
Since the flywheel will be spaced out 45 mm the starter motor will not reach the flywheel which is solved by using a flexplate from an automatic transmission.
